
Our tax organizer is designed to assist you in gathering important
information for your tax return. The organizer is in seven parts.
You can access any part by clicking on the Part Number and print
if necessary. You can drop off, fax, email or mail the completed
tax organizer along with any necessary documentation. If you fax,
email or mail our office will notify you of receipt of the documents.

dependents, estimated tax payments
Part 2 – Dependents,
interest and dividend income and stock sales
Part 3 – Itemized
Deductions – Medical expenses, deductible taxes, child care
expenses, interest deductions
Part 4 – Itemized
Deductions continued – Charitable contributions, education
expenses, miscellaneous deductions
Part 5 – Travel
Expenses, Moving expenses, Sale or purchase of home, Home business
use
Part 6 – Rental
and Business Income
